Insurance
For your convenience, we will file all claims and required paperwork with your insurance company. We recommend that you check with your carrier to determine your deductible and co pay, if applicable.
We accept most private health insurance plans and are in network with the following (if your insurance is not listed below, please check with us as we may be in network with them):
MOTOR VEHICLE ACCIDENTS
If you are involved in a motor vehicle accident we will file with your private health insurance and/or your auto carrier. We do not accept filing with a third party unless we can verify prior to the beginning of treatment they accept responsibility and will be paying the claims as they are submitted.
WORKER’S COMPENSATION
We accept all workers’ compensation insurance. If the claims are covered by workers’ compensation, the patient will have no liability. We do ask for private health insurance information for your protection so we can file with your private health insurance in case workers’ compensation would deny your claim.
MEDICARE
We accept Medicare assignment. This means we will accept what Medicare pays as payment in full, less any co-pay or deductible that is owed by you. If you have a supplemental policy we will file that for you also if you give us that insurance information.
Medicare requires that you have a physician order for your charges to be covered and processed. We will need a new order for physical therapy every 90 days if you are still in treatment. We will remind you when this new order is due. Maximum paid charges are $1,960.00 for 2016. We will let you know when you are close you are getting close to that amount.
